My Medical Benefits
New Hire Enrollment
New hire enrollment is the initial opportunity for a newly hired employee to select their healthcare benefits. New hires have 30 days from their date of hire to elect and enroll in healthcare benefits. This is the only time outside of open enrollment or a qualifying life event when employees can sign up for healthcare benefits.
Open Enrollment
Open enrollment is an annual period when all employees can review and make changes to their benefit plans for the upcoming year. During this set timeframe, employees can add, drop, or modify their coverage options. Changes made during open enrollment take effect at the start of the following calendar year, allowing employees to adjust benefits based on changing needs or to take advantage of new plan options offered by Definiti. Open enrollment is an active enrollment process, meaning employees must log in and select their healthcare benefits each open enrollment period.
Qualifying Life Events (QLEs)
A qualifying life event is a significant personal or family change that allows employees to adjust their benefit plans outside of open enrollment. If a qualifying life event occurs, employees have 30 days from the event date to contact PeopleandCulture@Definiti.com. Proper documentation showing the effective of date of change must be provided. The People and Culture team will guide employees through the steps for implementing these changes. Below is a list of qualifying life events for your reference.

Medical benefits are provided through Cigna Health Insurance. There are three plans available for employees to choose from. The HSA Gold and HSA Silver plans include a health savings account that allows employees to make pre-tax contributions to cover eligible expenses, which Definiti pays the account fees for.

My Telehealth Benefits
Employees enrolled under the Cigna medical plan have access to one (1) telehealth services – MDLive. Via phone or video conference you can consult with a board certified physician to obtain convenient care for minor conditions/symptoms. You are encouraged to register now, so you can avoid this process in the future when circumstances cause you to obtain the consult. Any payments for these services are automatically applied to the out-of-pocket maximum under your plan.
My Health Savings Account Benefits
Employees who elect the Health Savings Account may make a maximum contribution of $4,300 for single coverage and $8,550 for family. If you are over the age of 55, you may make an additional contribution of $1,000.
My Flexible Savings Account (FSA) Benefits
This benefit is provided through HealthEquity. An FSA allows you to set aside a pre-determined amount of money to pay for health care and/or dependent care expenses on a pre-tax basis. If you are contributing to a Health Savings Account (HSA), you can enroll in a Limited Purpose FSA to use for dental and vision expenses only.
GENERAL FLEXIBLE SPENDING ACCOUNT
Funds may be used for any medical or prescription expenses covered under your health plan that are not covered under another plan, and any other IRS-eligible expense (including most dental and vision out-of-pocket expenses). Funds are available in full on January 1st and must be used by December 31st, 2025. Maximum election is up to $3,300. Participants may roll over $660 into the following plan year.
DEPENDENT CARE ACCOUNT (DCA)
Dependent Care Accounts are a pre-tax benefit account used to pay for eligible services, such as preschool, summer day camp, before or after school programs, and child or adult daycare. DCA funds are available as they are deducted from pay. DCA funds not used in the plan year in which they were deducted from your pay are forfeited.
Maximum salary reduction:
- $5,000 for single employee or a married employee filing combined tax return
- $2,500 for a married employee filing a separate tax return

My STD Benefits
Definiti offers Short Term Disability (STD) coverage administered by Mutual of Omaha.
This benefit helps provide financial protection for insured members by promising to pay a weekly benefit in the event of a covered disability.
Your weekly benefit is 60% of your weekly pre-disability earnings, to a maximum of $1,000 per week. Your weekly benefit becomes payable the 1st day you are disabled for disability caused by accidental injury and after 7 days for disability caused by physical disease, pregnancy or mental disorder. You can only receive benefits for a maximum of 26 weeks.
Disability and Disabled mean that because of an Injury or Sickness, a significant change in Your mental or physical functional capacity has occurred, as a result of which:
- a) during the Elimination Period, You are prevented from performing at least one of the Material Duties of Your Regular Job (on a part-time or full-time basis); and
- b) after the Elimination Period, You are:
- prevented from performing at least one of the Material Duties of Your Regular Job (on a part-time or fulltime basis); and
- unable to generate Current Earnings which exceed 99% of Your Basic Weekly Earnings due to that same Injury or Sickness.

My Voluntary LTD Benefits
Definiti offers Long Term Disability (LTD) coverage administered by Mutual of Omaha.
This benefit helps provide financial protection for insured members by promising to pay a monthly benefit in the event of a covered disability.
- Coverage is paid 100% by the employee.
- Your monthly benefit is 60% of monthly pre-disability earnings, reduced by deductible income which include work earnings, workers’ compensation, state disability, etc. The maximum amount that can be received is $5,000/month. The waiting period for this benefit to become payable is 90 days.

My Voluntary Life and AD&D Benefits
Definiti offers Voluntary Life and Accidental Death & Dismemberment coverage administered by Mutual of Omaha.
- Coverage is paid 100% by the employee.
- Employees can elect a benefit amount for voluntary life and/or voluntary accidental death and dismemberment in $10,000 increments to a maximum of 5x your annual earnings up to $200,000.
- Evidence of Insurability is typically needed when an applicant enrolls above the non-medical maximum (or guaranteed issue) amount of $100,000, or enrolls outside their initial enrollment window, or was previously declined.
- Employees can elect voluntary life and/or voluntary accidental death and dismemberment on their spouse in $5,000 increments. The benefit must be the lesser of 100% of the Employee’s Voluntary Life benefit or $250,000. Any amounts over $30,000 will be subject to Evidence of Insurability.
- Employee can elect voluntary life and/or voluntary accidental death and dismemberment on their child in increments of $1,000 to a maximum benefit of $10,000.

My Parental Leave Benefits
Definiti will provide up to six weeks of paid parental leave to employees following the birth of an employee’s child or the placement of a child with an employee in connection with adoption or foster care. The purpose of paid parental leave is to enable the employee to care for and bond with a newborn or a newly adopted or newly placed child. This policy will run concurrently with Family and Medical Leave Act (FMLA) leave, as applicable. This policy will be in effect for births, adoptions or placements of foster children occurring on or after January 1, 2023.
This policy is subject to any leave periods governed by any applicable state family and medical leave programs. As a result, employees who reside in certain states will be subject to a separate parental leave policy applicable to the that state. Any such benefits received on account of a state family and medical leave program, shall offset any pay received from the Company during such leave.
Amount, Time Frame, and Duration of Paid Parental Leave
- Eligible employees will receive a maximum of up to six weeks of paid parental leave per birth, adoption, or placement of a child/children. Multiple births, adoptions or placements do not increase the amount of paid parental leave granted. No employee will receive more than six weeks of paid parental leave in a rolling 12-month period, regardless of whether more than one birth, adoption or foster care placement event occurs within that 12-month time frame.
- Each week of paid parental leave is compensated at 100 percent of the employee’s regular, straight-time weekly pay. Paid parental leave will be paid on a biweekly basis on regularly scheduled pay dates.
- Approved paid parental leave may be taken at any time during the 12-month period immediately following the birth, adoption, or placement of a child with the employee.
- In the event of an employee who has given birth, the six weeks of paid parental leave will begin at the conclusion of any short-term disability leave/benefit provided to the employee for the employee’s own medical recovery following childbirth.
- Employees must take paid parental leave in one continuous period of leave and must use all paid parental leave during the 12-month time frame indicated above. Any unused paid parental leave will be forfeited at the end of the 12-month time frame.
- Upon separation of the individual’s employment at the Company, they will not be paid for any unused paid parental leave for which they were eligible unless required by state law.
Eligibility
Eligible employees must meet the following criteria:
- Have been employed with the company for at least 12 months (the 12 months do not need to be consecutive);
- Have worked at least 1,250 hours during the 12 consecutive months immediately preceding the date the leave would begin; and
- Be a full-time or part-time, regular employee.
In addition, employees must meet one of the following criteria:
- Have given birth to a child;
- Be a spouse or committed partner of a person who has given birth to a child; or
- Have adopted a child or been placed with a foster child aged 17 or younger. The adoption of a new spouse’s child is excluded from this policy.
Requests for Paid Parental Leave
The employee will provide their supervisor and the People and Culture Team with notice of the request for leave at least 30 days prior to the proposed date of the leave (or if the leave was not foreseeable, as soon as possible). The employee must complete the necessary forms and provide all documentation as required by the People and Culture Team to substantiate the request.
